Delving into the Best Way to Deliver Services |
|
Communities are ever changing, and so are their health needs.
For that reason, we work with clinical leaders and others to revisit constantly the services offered by general practices. Partnership Health is one of the project directors of the Canterbury response to the Minister's drive to develop Integrated Family Health Centres.
We are part of a consortium helping support general practice to better connect and to look at ways to make services more accessible for their patients. By bringing together like-minded doctors, nurses and the multi-disciplinary teams that are becoming so vital to an effective community response, we are identifying what people's needs are, and making sure that services meet these.
The Canterbury District Health Board is also part of this project, looking at reconfiguring how services are delivered; exploring formerly hospital-based activities that used to be hospital-based that would be better delivered in the community, and working with us to change patient flows and models of care.
